چکیده

By CEsAR REYNAFARJE AND JOSE RAMOS H UMAN bartonellosis, or Carrion’s disease, is characterized by rapid development of severe hemolytic anemia in the acute stage of the disease caused by infestation of the erythrocytes by Bartonella bacilliformis. The bacterium described by Barton’ in 1905 is visible in the red cells at the acute stage, so that their examination makes the diagnosis very easy ( fig. 1 ) . Fever of sudden onset and irregular course, generalized bone pain, and general malaise are the most frequent symptoms which accompany the anemic phase. There follows a quiescent period in which the parasite disappears from the red blood cells, the anemia improves and, after a variable time, the second stage begins. This is characterized by the appearance in the skin and in some organs of nodules composed l)y histoid granulomata with capillary prolife ration, to which the disease owes its name, verruga peruana. Although studies had been made of this anemia since 1898, its hemolytic character was first described by Monge in 1915. Hemolysis was demonstrated by the elevated indirect bilirubin,5’ the increased phagocytosis of erythrocytes seen in autopsies,7.8 the indications of red cell regeneration, i.e., reticulocytosis and bone marrow hyperplasia,69’#{176} and by the increase in fecal urobilinogen.’#{176}’ Other characteristics have been described by Hurtado, Merino and Pons.#{176} However, some aspects of the hemolytic process in this disease could not be adequately studied until recent developments in hematologic technics became available. In the past three years we have carried out studies related to red cell survival, the mechanical fragility or parasitized red cells, metabolism of iron, immunohematologic observations, the site of sequestration of the red cells, and other investigations of the hemolytic process in this disease. Some of the findings have been presented in preliminary form.1213 In the present paper we are evaluating all the studies, as well as reviewing the anemia of human bartonellosis in general.

Cytokines and T-Lymphocute count in patients in the acute and chronic phases of Bartonella bacilliformis infection in an endemic area in peru: a pilot study.

Human Bartonellosis has an acute phase characterized by fever and hemolytic anemia, and a chronic phase with bacillary angiomatosis-like lesions. This cross-sectional pilot study evaluated the immunology patterns using pre- and post-treatment samples in patients with Human Bartonellosis. Patients between five and 60 years of age, from endemic areas in Peru, in the acute or chronic phases were i...

A case report with high bilirubinemia and hemolytic anemia during leptospirosis and a short review of similar cases

Background: Leptospirosis is characterized by very diverse clinical manifestations, which may range from flu-like subclinical forms to very severe presentations characterized by multi-organ failure, or to atypical presentations. One of its most aggressive presentations is Weil’s disease, characterized by jaundice, hemorrhagic phenomena and renal failure. Cases with high bilirubinemia over 30mg/...

Concurrent Atypical Hemolytic Uremic Syndrome and Autoimmune Hemolytic Anemia: a case report

Background: Atypical hemolytic uremic syndrome (aHUS) is a life-threatening and scarce disorder characterized by acute renal failure and disease, non-immune microangiopathic hemolytic anemia and thrombocytopenia, leading to end-stage renal failure or death, and consequently maybe accompanying by extra renal manifestations. Case report: We reported aHUS accompanied by autoimmune hemolytic anemi...

Autoimmune Hemolytic Anemia preceding the Diagnosis of Primary Central Nervous System Lymphoma

In this study, a 2.5-year-old boy suffering from a febrile seizure with normal laboratory tests and a history of immune hemolytic anemia was examined. Brain MRI demonstrated some tumors in the frontal, parietal, and occipital lobe that corroborated the pathology results of primary central nervous system lymphoma for the patient. The patient was treated with high- dose of Methotrexate. Our re...

Immune-mediated hemolytic anemia in cats referring to Veterinary Teaching Hospital of Tehran (2006-2007)

Immune-mediated hemolytic anemia (IMHA) is characterized by the destruction of erythrocytes or sometimes bone marrow erythroid precursors mediated by immunoglobulins (IgG, IgM), with or without complement (C 3). The main objectives of this study were to assess the laboratory test results of IMHA and to investigate its possible underlying causes in cats referring to the Veterinary Teaching Hosp...
